The 5 Modalities of TCM
Five canonical treatment approaches at the foundation of Traditional Chinese Medicine. Each works through a different mechanism; complete care often combines two or three modalities.
How TCM Treatment Works
Unlike Western medicine which often selects a single primary treatment, TCM thinks in terms of pattern differentiation: the practitioner identifies the underlying energetic pattern (qi deficiency, blood stagnation, damp heat, kidney yang deficiency, and so on), then selects from the modalities the combination most likely to restore the disturbed pattern toward balance.
Each modality has a distinct Yin Yang Five Element signature in our framework: acupuncture (Yang Metal, the precision needle), moxibustion (Yin Fire, gentle warming), herbal medicine (Yang Wood, the medicinal plants), cupping (Yin Water, receptive vacuum), dietary therapy (Yang Earth, nourishment). A skilled practitioner reads which signatures the body needs and prescribes accordingly.
TCM External Therapies (外治法)
The five modalities above are the textbook foundation of TCM, but actual clinical practice uses a much broader hands on toolkit. Tui Na (推拿 therapeutic massage), Gua Sha (刮痧 scraping therapy), ear seeds (耳穴埋豆 auricular acupressure), acupoint plaster (穴位贴敷 herbal patches), herbal steam (熏洗), paraffin wax therapy (蜡疗), plum blossom needle (梅花针 seven star tapping), and scalp acupuncture (头皮针) are the day to day external therapies practitioners reach for.
Several of these have crossed into mainstream Western wellness culture (facial Gua Sha is now a beauty staple; ear seeds for anxiety are widely shared; Tui Na has integrated into many physical therapy practices). Others remain specialized clinical interventions in modern Chinese hospitals.
